Do gutters really affect a foundation?

Yes, and it is one of the most common causes of foundation movement. Roof water discharged next to a foundation saturates the soil, and soil that repeatedly saturates and dries moves. The structure sitting on it records that movement.

How far should downspouts discharge from the house?

Far enough that the water does not return to the foundation. The right distance depends on grade and soil, which is why discharge is designed rather than defaulted.

Why do gutters overflow even when they are clean?

Usually because the system is undersized or improperly pitched for the roof area feeding it. It handles ordinary rain and fails in heavy rain, which is when overflow does damage.

Are gutter guards worth it?

They help where debris accumulation is a recurring problem. They are not a substitute for correct sizing and pitch.

Do you repair fascia when replacing gutters?
